Joining Our Troop

Joining our Troop is easy. We've attached here a few key documents for your review, download and filling-in.

 

All scouts must complete the Boy Scouts of America Youth Application

 

In addition, the following forms are required to participate in events. These cover a one year period and updates must be provided as they expire.


Annual Boy Scouts of America Activity Consent - This form provides parent permission for the scout to participate in all activities.

Annual Boy Scouts of America Health and Medical Record Parts A and B - This can be completed by the parents and is required for participation in all events. A copy of the current year insurance card is also required with this form.

The following forms are required as indicated. They also cover a one year period.


Annual Boy Scouts of America Health and Medical Record Part C - This section must be completed by a physician. It is required with Parts A and B for any event that exceeds 72 consecutive hours or when the nature of the activity is strenuous and demanding.
Annual Boy Scouts of America Health and Medical Record Part D -Required to be reviewed by all participants of a high-adventure program at one of the national high-adventure bases and shared with the examining health-care provider before completing Part C.


Troop 375 also gathers emergency contact, communication, and transportation information on the following forms.


Supplemental Contact and Scout Information - Required by all scouts
Driver Information - required for those parents that may help with transportation to scout events.
